Apply for Housing

The Referral Form for housing is to be filled out by a Referring Agency - this can be a church, government agency, or non-profit organization.  The Referring Agency must submit the following form on behalf of the applicant.  


To determine if an Applicant qualifies as a Guest in Beacon Hill, they must meet the following criteria:

  • Must be employed and working a minimum of 20+ hours each week
  • Must be motivated towards self-sufficiency
  • Must be sober (no drug or alcohol addictions)
  • Must be willing to participate in mentoring relationships
  • Must be homeless and make a <50% median income, as defined by HUD
  • No history of arson or attempted arson
  • Not on the State or Federal Sex Offender Registry
  • Applicant must agree to do assigned facility chores and abide by house rules

 All information provided on the Referral Form is strictly confidential and will only be used for the purpose of determining eligibility of an applicant as a Guest in Beacon Hill.  Submitting an application does not guarantee placement in Beacon Hill.
